Training Opportunities

INTAKE

"Intake" often represents the point of entry into the aging network for older persons or their caregivers. Most often a referring agency employee, such as a hospital discharge planner, social worker, or home health nurse, initiates an intake. Family members may also refer someone to Central Plains Area Agency on Aging for assistance, or the older person my self refer. An Intake Specialist asks a series of questions designed to obtain information about needs for assistance, and demographic and financial information. The intake is then forwarded to the appropriate unit within Central Plains Area Agency on Aging for services.

INFORMATION AND ASSISTANCE

Female on phone useing computer Information and Assistance (I & A) staff at Central Plains Area Agency on Aging maintains up-to-date resource information on aging services. I & A staff are responsible for informing individuals about available resources and referring them to appropriate service providers. Also included in I & A is education through community outreach.

CLIENT ASSESSMENT, REFERRAL, EVALUATION (CARE)

The CARE program was developed to meet requirements of mandated state and federal laws which state that prior to nursing home admission, an individual must receive an assessment of health and functional status and must be informed about community-based options. Based on that information, individuals and their families can make fully informed and educated decisions about long-term care. NUTRITION SITES

Meals served at nutrition sites - congregate meals - are provided by the Good Neighbor Nutrition Program. All meals meet federal nutrition guidelines, and are served five days a week at most sites. Over 198,900 congregate meals were served in 2002 at nutrition sites in the tri-county area.

NEWSPAPER

Active Aging Publishing Inc. publishes a monthly newspaper with unique content of special interest to older adults and mailed free to people 55 years and older living in Butler, Harvey and Sedgwick Counties. Approximately 50,000 households in the tri-county area received a copy of the Active Aging newspaper.

LEGAL SERVICES

Kansas Support Services for Elders (KSSE) - KSSE is a financial counseling service available to persons 60 years and older. Services are provided to allow seniors to live as independently as possible, by providing bill paying and other financial management services in their own homes or other residences.

Senior Citizens Law Project - The goal of the Senior Citizens Law Project is to provide services in a wide range of civil legal issues. An attorney or other person acting under the supervision of an attorney provides this service.

COUNSELING

Good Grief of Kansas provides a self-help grief support group for widowed persons in Butler county. The support group meets two times a month to help each person through the grief process. Emotional and educational support services will aid the program participants in dealing with grief and mourning as normal, appropriate, and desired reactions to the death of a spouse.

MEDICATION MANAGEMENT

Medication Management is a two-fold program. Services are provided individually to persons in their home environment. A registered nurse will come to the home and discuss the consumer's medications with them to ensure that the consumer understands what the medication is for, the medication is being taken correctly and to facilitate any communication that is needed between the physician and the consumer. The second fold of the program is group education presentations. This service is provided at senior centers, senior focused housing or churches. Two or three medications are discussed in depth at each presentation with a question and answer period following presentation. The purpose of the presentations is to prevent incorrect medication and adverse drug reactions.

ROUTINE HEALTH SCREENINGS

Routine health screenings are provided at various locations. Locations can include senior centers, senior focused housing and churches. Activities to detect or prevent illnesses can include screening for high blood pressure, hearing, vision, glaucoma, diabetes, and cholesterol.

HOME INJURY EDUCATION

Home injury education presentations are designed to provide information about injury prevention in the home environment. The programs will include information about how to prevent falls and fractures. Presentations are provided in various locations that may include senior centers, senior focused housing and churches.

HEALTH PROMOTION

The Health Promotion program is designed to educate consumers in a group setting about chronic disabling conditions (including osteoporosis and cardiovascular disease) prevention and reduction of effects, alcohol and substance abuse reduction, smoking cessation, weight loss and control, and stress management. Presentations can be held at various locations including senior centers, senior focused housing and churches.

PHYSICAL FITNESS

Physical fitness and exercise programs include aerobic and other exercise. Currently this service is provided in Butler County only. To find out more about classes please call the Butler County Department on Aging at (316)775-0500.
