Training Opportunities

Case Management

Case managers are responsible for ensuring that older adults seeking alternatives to nursing home care will be provided with information on appropriate community-based long-term care services so that informed choices can be made. Case managers also provide advocacy services.

Case management services target persons aged 60 and older who are residing at home and who are at risk of institutionalization. A case manager performs an assessment of an older individual's physical, psychological, and social needs, develops a tailored plan of care of services to meet those needs, sets up services, and monitors service delivery.

Home and Community Based Services (HCBS) Frail Elderly Waiver

The Home and Community Based Services (HCBS) Frail Elderly Waiver program serves persons 65 years of age and over. To be eligible for services, an individual must be a Medicaid recipient and meet a functional needs assessment. In-home services through the HCBS-FE program include:

Health Care Attendant One Assistance with housekeeping tasks, such as cleaning, Level One meal preparation, and laundry.
Health Care Attendant Two Physical assistance with tasks such as bathing, Level Two dressing, eating and toileting.
Personal Emergency Electronic device is worn and when button is pushed it signals a responder for help.
Respite Care Provides temporary relief for a caregiver and includes supervision and limited physical assistance.
Adult Day Care Supervised setting for older adults who cannot be alone.
Sleep Cycle Support Provides assistance in the home during sleeping hours; may include help with toileting, transferring in and out of bed, and reminding to take medications.
Wellness Monitoring Health assessment, education, counseling, and monitoring of treatment program prescribed by health care professionals by a Registered Nurse.

Senior Care Act

The Senior Care Act program serves persons 60 years of age and over. Program participants must meet a functional needs assessment and be willing to share in the cost of their care. Consumers are required to pay from a donation to 100% of the cost of services based on monthly household income and liquid assets. In-home services through the Senior Care Act program include:

Homemaker Service Assistance with housekeeping tasks, such as cleaning, meal preparation and laundry.
Attendant Care Physical assistance with tasks such as bathing, dressing, eating, and toileting.
Personal Emergency Electronic device is worn and when button is pushed it signals a responder for help.
Respite Care Provides temporary relief for a caregiver and includes supervision and limited physical assistance.
Adult Day Care Supervised setting for older adults who cannot be alone.

Older Americans Act IIIB

The Older Americans Act IIIB In-home service program serves persons 60 years of age and over. Program participants must meet a functional needs assessment. Participants do not need to meet any financial criteria. In-home services through the Older Americans Act program include:

Homemaker Service Assistance with housekeeping tasks, such as cleaning, meal preparation and laundry.
Attendant Care Physical assistance with tasks such as bathing, dressing, eating, and toileting.
Respite Care Provides temporary relief for a caregiver and includes supervision and limited physical assistance.

Minor Home Repair

The Minor Home Repair program provides services to residents 60 years of age and older. The program addresses requests relating to access, safety and health issues confronting older adults. Minor Home Repair assists with wheelchair ramps, bathroom modifications, “emergency” plumbing, and “emergency” electrical problems.

Program participants do not need to meet functional or income criteria. Eligible consumers must reside in and own the property needing repairs, and not have any outstanding liens or owe back taxes. Renters must have written authorization from landlords. Participants are asked to contribute toward the cost of service.

Home Delivered Meals

Home delivered meals are provided to individuals, age 60 or older, who are ill or for any other reason are homebound. Home delivered meals are provided by the Good Neighbor Nutrition Program, Senior Services, Inc., Newton Meals on Wheels, and El Dorado Meals on Wheels. Over 292,487 meals were delivered in 2002 in the tri-county area.
